Project Proposal Form

Fill up the PPF

1.  Make a copy of the Project Proposal Form. 

2.  Follow the instructions in the first page of the form and fill up the required information. The sections that need to be filled out will vary depending on the project.

3.  Once finished, ask for feedback from the overseeing Department and Office Heads, and change the information in the form if necessary.

Obtain endorsements

4.  Once DHs and OHs are ok with the form, send an email to the AMS President, chrislyn.manero@student.ateneo.edu, and AMS Moderator, egarcia@ateneo.edu, for endorsement. You may also add other people to the email thread depending on their involvement in the project. 

5. Once endorsed by the president, keep a screenshot of the endorsement containing her name, student email address, and date of endorsement. Use the file name AMS_PPFPRESENDO_Name of Activity.jpg for the screenshot.

6.  Once endorsed by the moderator, keep a screenshot of his endorsement containing his name, Ateneo email address, and date of endorsement. Use the file name AMS_PPFMODENDO_Name of Activity.jpg for the screenshot.

Submit the PPF

7. Upload the form and the proof of formal endorsements through the PPF Submission Form on OSA's online portal. Note that the online portal only accepts submission on certain periods, Monday to Friday (8:00AM-5:00PM) and Saturdays (8:00AM-12:00NN), with the exception of holidays.

8. Schedule an Individual Consultation with the AMS Formator (Sir Janlo, jcapuyan@ateneo.edu) upon submission of the PPF. At least one of the Project Heads and at least one of the Department Heads must be present.

9.  Wait for Sir Janlo's approval for the project, and answer any of his questions regarding the project.
